1997 Ram Van 2500
I need to replace the outer-right tie rod and sleeve. I may try this on my own. Do I need to raise the van up to do this? Is it just unscrewing nuts and bolts, or is there more to it than I realize?

Being able to raise the vehicle to remove the tire would greatly simplify the task. That aside, you should be able to change it on the ground. As you turn the old part off be sure to count the number of turns and reinstall the new part the same number of turns.

You probably need a tie rod pickle fork or press to remove the old piece.
