'95 Chrysler Concorde Smog Issues

Hi guys, I just purchased my first car and I probably made a mistake purchasing this but no going back now...anyways, I went to go get it smogged and I figured I was going to fail but this is the results.

Vehicle Inspection Report:

So I failed because of high NOx emissions, and also because the PCV was modified. His note said "Failed for using Coolant Heater hose in a crankcase system."

Crvett69 has informed me that this is the hose he is speaking of:

He said I should find a "vacuum hose" at a parts store that would be large enough to fit.

Also, how would I go about lowering the NOx emissions to acceptable levels? Crvett69 has gave me the advice to try cleaning/replacing EGR valve as well as possibly replacing the catalytic converter.

Thank you for helping me out Crvett69 and anyone else willing to give me some advice!
