Here is the answer from the GM 94 Cutlass manual:

1) Depress the SET button. The display will show "000".
2) Enter the six digits of the user-selected code following these steps:
a. The first three digits (0-999) of the user code are entered by depressing the SEEK and SCAN pushbuttons. Depressing the SEEK button will cause the left digit to increase at a rate of about 5 counts per second. Depressing the SCAN button will cause the two right digits to increase. The display will show the numbers as entered.
b. Depress the AM/FM pushbutton. The display will show "000". (The selected code will not be shown.)
c. Enter the second three digits of the code according to the instructions in "a." above.
3) Depress the AM/FM button. If the unlock sequence was successful (the numbers matched either the user-selected code or the factory back-up code), the clock will be displayed, indicating the unit is operational. If the display shows "LOC", the numbers did not match and the unlock sequence was not successful.
Important-After an initial 8 tries, the unit will time out for an hour and will time out for an additional hour after every three sequential tries.

If this method doesn't help you, you will need a dealer to contact the number in the manual and obtain the unlock code. This requires doing an inquiry from the radio (listed in the manual) for a six digit code, having the dealer contact the phone number in the manual, punching in their dealer code and the code obtained from the radio. Then they can get the factory code, and reprogram the radio so it is operational. Sorry for the long post-just giving the facts for your case.
