windstar intermittent start problem

I own a 98' Windstar that has an intermittent start problem. The engine cranks, has spark, and will not run until I put fuel in the throttle body. Once started this way it runs like new and drives just fine. The engine light has not come on. The IACV, reference voltage, differential egr pressure transducer, map sensor, throttle position sensor have checked out ok.
I would like to hear your suggestion as to what the problem may be.

Does manually holding the gas pedal partway down while cranking make a difference?
If so, this is a classic sign of a stuck IAC valve (I know - it checked out fine for you, but mine only stuck some of the time).

Problem found and fixed. The problem was the fuel pump.
After replacing the camshaft position sensor the problem still persisted. I then checked and found there was no fuel pressure in the fuel manifold on these occasions. After checking the electrical circuit I replaced the fuel pump and have not had the problem reoccur for over a week now. Why the fuel pump had this intermittent no-run only when starting the engine and only once or twice a week is a mystery to me. I hope this helps someone out.
