It's a pretty easy process but the process has to be completed with in 30 seconds start to finish or it wont accept the new remote. Here is a link to a site with the instructions for both RS3200 securitys system remotes and Keyless entry remotes. I hope it helps.

Highlander did not have the RS3200 system.

1. Start with the following conditions: You are in the drivers seat.
a. Key is out of the ignition
b. Driver door is open (all other doors closed)
c. Driver door is unlocked (other doors don't matter)
2. Insert and remove the key from the ignition twice within 5 seconds.
3. Close and open the drivers door twice within 40 seconds.
4. Insert the key into the ignition and remove it.
5. Close and open the drivers door twice within 40 seconds.
6. Insert the key into the ignition.
7. Close the door
8. Turn the key in the ignition on and then off.
9. Remove the key from the ignition.
10. The power locks should automatically cycle once to indicate that you have successfully entered program mode.

a. press both transmitter lock and unlock buttons SIMULTANEOUSLY for a duration of from 1 to 1.5 seconds.
b. within 3 seconds, press the transmitter lock button for more than 1 second and release.

Just wanted to add my personal experience on my '01 Rav4:
I've followed quite a few guides which agree with this one, & had tried umpteen times following the instructions verbatim. It just so happens I accidentally did steps 4 & 5 twice & was the only time the car entered programming mode. Once I figured out what I did, this was a fully repeatable sequence.
In other words, the Toyota TSB instructions is WRONG! Since all of these vehicles are based on the same RS-3200 system, I doubt other models listed would have luck without my "mistake" above. Just don't want anyone else pulling his hair out during the night!

Have you tried this site? I'm not sure if it has anything different but I know they have programming instructions and even installation instruction for full factory security systems.

I used the instructions for my '03 Tundra and they worked fine.

I followed the instructions posted by mrlajoie for my 2005 Highlander Limited and they worked fine to program a new genuine Toyota keyless remote.
Note that these are the "Add Mode" instructions posted at the website indicated above by Jeff2004.
